If you are seeking enrollment in our Donated Care Program, we will need to receive a referral from your primary care provider. You cannot apply directly to the program. If you do not have a primary care provider, visit a free or low-cost clinic.

Donated Care Patient Responsibilities

1. Attend all of your appointments on-time.
Your first appointment is crucial. Late arrival or failure to attend will result in disenrollment.

2. Call our main line to report all follow-up appointments, lab work, and imaging orders.
If we are unable to answer the phone, leave a voicemail with your name, date of birth, and a brief description. When reporting appointments, provide the appointment date, time, location, and provider.

3. Maintain good communication.
Notify us of any canceled appointments or changes to your appointment. Inform us if you get lab work done or have an imaging order. Inform us if you obtain medical insurance, move outside Pierce County, experience a change in income, or need to update your phone number, email address, or mailing address.
